0290 (2) Licensor observed the toddler may transition to a sleep mat due to the toddler folding the pack and play mattress and manipulating the mattress in an usafe manner. 0290 (3d, 3e) Licensor observed the pack and play mattress did not have a fitted sheet in place and the mattress did not appear to be sanitized or cleaned after use (signs of bodily fluid markings).
